Dislocation after Total Hip Arthroplasty: A comparative matched study -A comparative matched study-

Abstract
Purpose : To evaluate the factors that contribute to dislocation after total hip arthroplasty and th influence of dislocation on the result of total hip arthroplasty.

Materials and Methods : We analyzed 351 hips that had undergone total hip arthroplasty. A matched comparative study was performed for the dislocated group (16 cases) and the non-dislocated group (16 cases) with regard to original diagnosis, patient factors, and prosthesis type. The 32/351(Ed-confirm which) cases/patients were followed for more than 2 years (average 44.6 months).

Results : The incidence of dislocation was 4.6% (16 of 351 cases). Mean patient age was 60.1 years in the dislocation group and 52.4 years in the control group (p<0.05). Statistical analyses of size, diameter, inclination and anteversion of the acetabular cup and of the leg length discrepancy revealed no significant differences (p>0.05). The Harris hip score was 87.25 in the dislocation group and 90.13 in the control group (p=0.016). The rate of linear cup wear in the two groups was 0.15 0.05 and 0.160.04 mm/year (p=0.305), respectively. The loosening of implants did not occur in these two groups.

Conclusion : We think that the most important causative factors were related to patient compliance as well as the operative factors or the prosthesis type in this study. The wear rate of the polyethylene liner and the loosening of implants showed no significant difference between the two groups. However it is necessary to carefully follow-up those patients who show hip dislocation.
